Introduction
In Sweden there are today 89 National medical Quality registers. Their chief purpose is activity analysis and continual work for clinical improvement. The job also includes clinical research. The registers should be seen as prospective observation studies well supplementing randomised clinical studies in the area. The great strength of the register is its generalisability, which contrasts with the many limitations of randomised studies such as ‘performance bias’ which in these types of study often reflects a medical or surgical intervention at a specialised unit. A further strength is the ability to analyse statistically unusual and serious complications.
